Passport Gender Marker Change
If you plan on traveling out of the country you obviously must hold a current US passport. For many of us which hold a current passport we need to update our gender status. The process isn't that hard, you will need to do a few things

1) Medical Certification
A signed, original statement from a licensed physician must be on office letterhead and include:

Physician’s full name, address, and telephone number

Medical license or certificate number

Issuing state or other jurisdiction of medical license/certificate

Language stating that:
-
He or she has treated you, or has reviewed and evaluated your medical history
-
You have had appropriate clinical treatment for transition to male or female, or are in the process of transition to male or female

2) A new passport photo that resembles your current appearance
UPS, CVS, Walgreens, Rite Aid and many other drug stores provide passport services ranging from $7-$15 and it only takes a few minutes. The USPS takes passport photos too but hours are usually limited so I dont suggest it.

3) Proof of legal name change if applicable

4) Copy of current ID that resembles your current appearance
​
5) A check for $110 made out to the US Treasury Dept ( this is as of 2020 )

6) The DS-11 Form Filled Out

I suggest you just print out the form and fill it out by hand. If you try and use their form filler it automatically defaults you to the DS-82 form which is the incorrect form even if you choose the DS-11 form. Click HERE to download the DS-11 form (valid as of 4/2020 )

7) Include Your Old Passport
You must send in your old passport with a photocopy of the information.

8) Bring your forms down to your local passport office. The forms can't be mailed.
